Pharmacists possess unique skills, making them great generalists and exceptional specialists in GP Practices and PCNs.
Consider Pharmacist-led clinics
ARRS funding has dramatically increased the number of clinical pharmacists working in GP Practices and Primary Care Networks (PCN). They have been used to complete Structured Medication Reviews (SMR), process clinic letters, complete clinical audits and generally help with medication-related tasks in practice.
Pharmacist-led Clinics
In short, they have been used as generalists but we are missing a trick to date on pharmacist-specialists. Pharmacists often focus on a particular clinic area during their Independent Prescribing training, and many take that a step further after securing IP status to increase their knowledge, confidence and competence to support patients with long-term conditions. Some pharmacists with experience in secure establishments like prisons or secure hospitals have very high levels of skill in helping patients with mental health and substance misuse challenges and pharmacist-led clinics in these areas are always well received by patients.
Medacy, with its extensive team of prescribing pharmacists, is uniquely positioned to provide specialist support to PCNs in specific conditions, such as Chronic Pain, Mental Health, Type 2 Diabetes, Heart Failure, Rheumatoid Arthritis, and Dermatology. Our services are designed to alleviate the burden of managing these conditions, allowing you to focus on other aspects of patient care.
Working across multiple PCNs
Whether your Practice has a high demand for a full-time pharmacist specialising in a specific condition like Chronic Pain, or if you have a more varied workload, Medacy can adapt to your needs. As a CQC-registered, pharmacist-led clinical services provider, we can efficiently manage our team diaries and allocate clinical pharmacists across multiple PCNs, ensuring that their time and knowledge benefit several practices.
Not every Practice or PCN may require additional support for conditions like Chronic Pain or Mental Health, and that’s perfectly fine. However, it’s important to note that most practices and PCNs do face challenges with at least one long-term condition. In such cases, Medacy provides the necessary support and expertise.
Support available in most LTCs
Pharmacist-led Clinics can be provided in the following long-term conditions::
- Chronic Pain
- Mental Health,
- Asthma,
- COPD,
- Hypertension,
- Rheumatoid Arthritis.
- Heart Failure
- Diabetes
- Atrial Fibrillation.
These pharmacist-led clinics can be taken further by working with Physio and psychologists to support Chronic Pain patients or Mental Health Practitioners for patients with anxiety or depression.
When agreeing to provide additional focused support for PCNs Medacy looks for a minimum six-month agreement to launch, embed, evolve and evaluate the support.
