At Medacy we believe that many of the changes seen in the way Clinical Services are delivered during the COVID-19 pandemic will stick. The NHS has traditionally been the proverbial oil-tanker that takes forever to change direction, but the last few weeks have seen this change. It’s one of the very few positive things to come out of the pandemic. Traditional silos have been either toppled over or merged and projects have been delivered at pace.

Problem

A CCG  identified a project around patients taking Warfarin during the pandemic. Warfarin patients need regular monitoring and this involves regular visits to either the hospital or GP Practice. This is clearly not ideal during a time when we are encouraging social distancing and self isolation. Medacy have a solution.

Medacy’s Solution – Clinical Hubs

Medacy’s model of pharmacist-led clinical service is designed around the needs of patients rather than professionals or NHS real estate. It offers an ideal model to deliver clinical services at pace and is ideally suited to Pandemic change programmes. Medacy’s agreement will see  patients changed from Warfarin to a NOAC/DOAC, where clinically appropriate. The project is delivered by Independent Prescribing Clinical Pharmacists managed by Medacy. The project has involved liaising with the Anticoagulation Team at the secondary care trust, negotiation with the CCG and discussions, with GP federations, Primary Care Networks (PCNs) and individual practices. It has also involved, recruiting and training six clinical pharmacists. This has been done within a week and will reduce COVID-19 related risk to patients and Healthcare Professionals

The project will be delivered over three months using the Medacy Clinical Hub model of care. The Clinical Hubs are a patient centred model for delivering care with any long term condition Most importantly the Hub model offers a flexible way of delivering great care to patients. 

Outcomes 

COVID-19 is awful and we all look on in horror as more and more patients succumb to the illness. We watch in awe as front-line NHS staff battle to save them or give them dignity and love in their last moments. The changes we are delivering for these anticoagulation patients will decrease risk for patients and NHS staff. They will also free up clinical time to spend more time with patients.

In or out of Pandemic, the same applies. The Medacy Clinical Hub can deliver improved care to patients with Rheumatoid arthritis, Diabetes, Asthma or COPD. It offers a new way for Clinical Pharmacists to work and offers a possible glimpse of the future of Pharmacy Practice.
